Waiting times for replantation surgery: China, the US, Canada and the UK compared

The clock is not the same for every tissue

A finger with no muscle can survive cool ischaemia for many hours; a limb with muscle — forearm, calf, thigh — begins irreversible damage far sooner. That asymmetry is why "wait a few weeks for a specialist slot" is a reasonable sentence for an elective procedure and an unacceptable one for a major limb injury.

So the question is not whether a health system is good. It is: from the moment you are injured, how many days until a microsurgeon opens the vessels?

Typical times from first contact to theatre

The pattern is consistent: emergency care is fast everywhere, scheduled reconstruction is fast almost nowhere.

Where the weeks actually go

  1. Referral. Your family doctor must be convinced the case is surgical, then finds a hand surgeon who accepts referrals.
  2. Consult queue. The first appointment is often the long one — 4 to 20 weeks.
  3. Imaging and clearance. MRI, vascular studies, cardiac clearance, each with its own queue.
  4. Theatre scheduling. Even after approval, an operating room with microscope time and a free bed has to align.
  5. Insurance authorisation. In some systems this is the single longest block.

Only step 4 is genuinely about surgical capacity. The rest is administration — and administration is what an international patient pathway is built to bypass.

What you can compress yourself

The uncomfortable part

Waiting lists are not a moral failing of any single system — they are the arithmetic of finite theatre time. But arithmetic has consequences: a finger that could have been saved in week one is sometimes a revision amputation in month four. If you are reading this while deciding whether to wait, the honest advice is to get a written opinion now and keep your options open. It costs you nothing to know.
